ABSTRACT
The Renal Group of Israeli Dietetic Association – A.T.I.D, prepared nutritional guidelines of chronic kidney disease without dialysis, for community dieticians and physicians. Proper dietary treatment and follow up, as mentioned in the K/ DOQI OF NUTRITION, may significantly delay the progression of renal failure, which stressing the importance of preparing proper guidelines for dieticians. The guidelines contain several sections: assessment of risk factors for CKD and CHD, nutritional assessment, indications for starting the renal diet, and the nutritional recommendations for different types of renal disease: nephrotic syndrome, diabetic nephropathy, proteinuria, hypertension and dislipidemia. The guidelines contain recommendations for the frequency and duration of nutritional treatment and follow up. It includes new food exchange groups with new data on the exact amounts of potassium and phosphorus contained in each product, providing an important instrument for both dieticians and patients. In the existing guidelines of the ADA this information is not included. This new approach improves the variability of the food choice. The vegetables and fruit exchange groups are divided into four sub-groups (low, medium, high and very high potassium), with a reference to the exact value of potassium for each item. The milk exchange group is built in accordance with Israeli milk products standards, each portion containing an average of 7g of protein. The exact content of potassium and phosphorus is specified. In the meat, fish and cheese exchange group each portion contains an average of 70-100 mg potassium and the exact content of phosphorus is specified. The bread exchange group is divided into two groups – one for diabetics and one for non-diabetic patients. The fat exchange group is divided into 3 sub-groups, according to the content of saturated, mono- and poly-unsaturated fats.
